COSTUME POLICY

•Costume Payment is $75 per class. Due 3rd week of October and will be paid by cash, check or credit card.
•Students or families with more than 3 costume payments may pay in three installments October, November,
and December by cash, check or credit card.
•The Credit Card on File will be charged for costumes on 3rd week of October. Installments will be charged
each month on the 20th of the month.

TUITION POLICY

•Submit all payments so you retain a receipt. We will not be responsible for cash.
•Schedule drops, adds or changes may cause your tuition amount change.
•We accept cash, check & credit card for payment of tuition
•Tuition will automatically be charged to the credit card on file on the 1st of each month October – May. You
can request that your card be charged on the 15th of the month. You must request this change in writing or
by stopping in the DDD office so your account can be updated.
If your credit card payment is declined we will run the card again after 48 hours. You can make a cash
payment in the studio office within 5 days of the declined transaction. After 5 days your account will be
charged a $35 late fee.
•$35 NSF charge on ALL returned check transactions
•No credit is given for missed lessons. Make up classes are available.
•Tuition is divided into 8 equal payments from October to May. The school year consists of 32 weeks of
classes

COVID 19 POLICY (updated 9/22/22)
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk
that my child(ren) and I may be exposed to or infected by COVID-19 by being in DDD. I voluntarily agree to
assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself
(including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or
expense of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)s
attendance at DDD. I agree and understand that my child(ren) or myself will follow all of the safety protocols
outlined by DDD (use of hand sanitizer, frequent hand washing and keeping sick children home). I further
understand that DDD is following “best practices” in accordance with the local government and health
departments and my child(ren) and myself must follow written protocols or risk dismissal from DDD.

On my behalf, and behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless
DDD, its employees, and representatives, or and from the Claims, including all liabilities, claims, actions,
damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this
release includes any Claims based on the actions, omissions, or negligence of DDD, its employees, staff, and
volunteers, whether a COVID-19 infection occurs before, during, or after participation in any Studio activities.
I will not send my child/child(ren) to DDD with any aware symptoms. If he/she tests positive, I will
communicate with DDD and we will advise each family on a case by case basis depending on the
circumstances. DDD is following these guidelines as of 9/22/22 – Upon a positive test, student or parent will
quarantine for 5 days and can return to DDD on day 6 with a mask. Members of household can continue
regular activities and are asked to mask for 10 days.
Liability agreement
All students and the parents/legal guardians are aware of possible physical injury that may occur during
dance classes, performances, and/or rehearsals and are willing to assume those risks. It is agreed that
students and their parents/legal guardians will not hold DuHadway Dance Dimensions, LLC its directors,
officers, and/or employees liable for injuries sustained while in attendance or while participating in any
DuHadway Dance Dimensions, LLC activity. We give our permission to DuHadway Dance Dimensions, LLC
to use pictures and videos of our child for advertising purposes and in social media.
The responsible party agrees to all of the above terms stated here and in the studio policies.
