Terms / Agreement

  1. A $15.00 per student or $20.00 per family registration fee is due with this form.
  2. Recital Costumes are $50 for each class. Payable by postdated check dated December 1.
  3. Tuition by post dated checks OCTOBER-MAY to be cashed on 1st or 15th of each month or whole year payment (10% Discount) postdated for September 1 or two half year payments (5% Discount) TWO checks postdated for Sept. 1 & Jan. 1 or 15.
  4. All tuition and costume fees are due upon receipt of your schedule.

The responsible party agrees to all of the above terms stated here and in the studio policies and is responsible for all fees including, but not limited to, costumes and tuiition. All students and the parents/legal guardians are aware of possible physical injury that may occur during dance classes, performances, and/or rehearsals and are willing to assume those risks. It is agreed that students and their parents/legal guardians will not hold DuHadway Dance Dimensions, LLC its directors, officers, and/or employees liable for injuries sustained while in attendance or while participating in any DuHadway Dance Dimensions, LLC activity. We give our permission to DuHadway Dance Dimensions, LLC to use pictures and videos of our child for advertising purposes.
